Warrenty return via e3d

Hi guys,
I purchased a duet 2 wifi from e3d two weeks ago. Unfortunately the E0 stepper driver has failed with a visable damage to the driver.
E3d have asked me to post hear in order to obtain a warranty form.

I approve the exchange of that board under warranty. But please check the wiring to that motor for possible shorts or intermittent connections.
