Heater Core issues?

I cannot figure out what is going on. All of the wiring appears good.
This doesn't happen all of the time, but as now it has happened at least 4 or 5 times this week, and last week and before, never happened. Most of the time this occurs after printing a while, but once it happened during heat up to 260c. I generally print between 225 and 245c depending on plastic.

I was running 2.02 RC4 but put 2.01 back on thinking it could have caused it... nope

Double check the heater cartridge clamp is not loose, check resistance cold, and if your multimeter can cope check what current it draws. If all them are good with no sign of glitches (perhaps a scope on the +ve side?) the run an auto tune on the heater (making note of previous PID values). If you see any roughness in the temperature plot then you make either have electrical noise issues or a dying temp sensor.

I've just cleaned and tested a E3D V6 Lite and I think (/hope as I've ordered another) the temp sensor in that has died. If you can get it up to temperature try moving the head around (via dwc rather than hand) and watching the temp plot for glitches. Likewise give the wires a wobble near the connectors.

Edit: Also worth checking V_IN voltage when the heater is on. You may be pushing the PSU a little hard.

Edit2: Worth checking that you see a permanent voltage on the +ve side of the heater, and a voltage across the heater when the controller commands for heat. Also ensure heater LED indicator lights.

Sorry for the delay. it appears just complaining about the issue to you guys fixed it.
Before I asked for help, I checked all the connections and re-tightened the connections on the Duet2 I went through and checked all the wires 3 times as it kept happening. Posted a comment and no problems (so far) since.

Saying that, I have never tuned the heater. I only had to change up the configuration on the heater a few times until I figured out how to properly use the configurator, and specifics of the thermistor. I know should have done it right after I setup the printer. Haven't had it up for very long, and planning to do a lot of upgrades/replace parts over Christmas.
