After running a simulation of a print, the M122 report shows: Underruns: 2881, 0

When I print these files, I never typically get underruns (that I've noticed), but at high speeds I do see some hiccups.

I assume this is merely because the simulation is going as fast as possible, and my high-resolution gcode file is overloading the system.

Is this acceptable behaviour and does it affect simulated time?

Your diagnosis is correct. The simulated print time may be slightly longer than the actual print time.
