Cooling fan for Titan Aero

I just added a cooling fan and got my first PLA jam in my heat break on my first print (temp tower)…

I am guessing that the fan was possibly too high on the nozzle or the block.

Does anyone have a good model to go with the basic bracket (https://e3d-online.com/titan-motor-bracket)

I printed the one that Phaedrux linked, it works OK as long as you arent using dual extrusion.
