Smoking U3

Hi!
We were setting up a Duet2 Ethernet the other day which a few seconds after plugging it with a USB cable (nothing else attached) started to crackle and let out a fair amount of magic smoke from the U3 chip...
@dc42 Could this perhaps be a faulty chip? I read that there has been some faulty U3 chips in the past, but those threads were from quite a while ago. -

as in no 12v or 24v supplied? and from your wording presumably its a new and never before used board?

Yes, just the USB-cable (same way that we've done with 20 or so other boards with no issues), and the board was straight out of the box. It had been sitting on our shelf for a few months before we commissioned it, but it has been sitting in its original packaging in an untouched box for that time -
I think the past failures were with Vin applied, but while we wait for the Duet people to chime in, any chance for a nice close picture up of the chip?

That definitely looks like a molten slag heap instead of a functional U3. Sorry about that.
Where and when was the Duet purchased?
