Tool Heater Faulting - Just Started

I am having issues with my hotend heater. Everything seemed fine yesterday when printing.
Turned on the printer today and set the tool to 240. I noticed the element was glowing red and stopped everything. It looks like, even when I try and put the hot end to 200 it can't hold the temp stable.
When I tried to go to 240 after restart it started glowing again and hit about 280 and I stopped everything. Now the heater faults when trying to get it up to 150.
Thermistor seems to be working. Wondering if it could be the PSU? Thoughts?

The element could have gone bad. Have you run a PID autotune on it or are you bang bang?

I ran an autotune on the bed, I don't think on the hotend. It has been running nicely for about 4 months now with no issues. Just out of curiosity, what is bang bang?

bang bang is a different way of controlling the heating. I personally used PID for the bed and the hot end.
Id start by trying a PID autotune on the hot end and see what happens.
M303 H1 P1 S240 (this assumes Heater 1 is your hot end, you want full power PWM, and 240 is your target temp)
See what that reports when it is done, and then do M500 to save the results to your config-override.g if all is well.

Bang bang is switch on/off regulation mode. When temperature is below a set value heater is switched on and it is switched off when above this value possibly with some hysteresis. Regarding your case it looks like some problems with heater insulation when it is heating so its resistance get lower and power goes up. Can you check heater's resistance when it is hot?

So I tried the autotune and it failed.
I was just poking around the nozzle and looking at things. The barrel of the heater was pretty loose, I tightened that up. The autotune still failed, but interestingly enough everything is back to normal after tightening up. It looks like everything is stable.
Question is, why is it still failing the autotune.. a bit concerning. Any thoughts?
