Revo Roto and Z Probe Spacer

In the Revo Roto Toolboard documentation, the picture shows the probe mounted to a spacer with the board.

My question is: Where can I find that mount? The Documentation includes a link to the github which has a basic spacer, but not the one shown in the documentation. Any assistance would be appreciated!
