If you have no luck with MotionOS, try VideoStream. It streams via a url that can simply be inserted into DWC.

Your homing files are set to use an endstop, not a probe.

G1 H1 Z-266.5 F6000 ; move Z down until the endstop is triggered
G92 Z-2.5 ; set Z position to axis minimum (you may want to adjust this)

Replace those lines with something like

G1 X150 Y150 F6000 ; move the center of bed
G30 ; Probe the Z axis

When using external 5V power there is a diode between the external 5V input and the 5V bus on the Duet. That's why you only see 4.75V on the expansion connector. It should be enough for a BLTouch, but if in doubt you could connect the +5V wire of the BLTouch directly to your 5V external power input.

We could have been half way through a board exchange by now.

Take extra care on wiring the replacement. Shorting the pins of the drivers are a good way to damage them.
