It is highly unlikely that the SSR has failed since it is not stuck in the 'on' position. It is way more likely that there is a wiring error.
Are you aware that the positive line will always be high no matter if the heater is on or off?
Simply wiring the negative to ground will cause all these issues.
Verify the ruting of the two control lines.

@dc42 do I remap a pin to H0 or do map a different heater to "bed"? I'm kinda lost as to which direction to go.

If you are using RRF2 then use a different heater number. Use M140 with an H parameter in config.g to tell the firmware which one.

It's very usual to have a dud bed heater mosfet on a Duet. Is the bed heater LED on the Duet permanently illuminated too?

@gnydick : If I look at your photo, I believe you made a wiring error, you have wires labeled "BED-SSR" connected to the VIN connector and wired VIN to the Bed output connector!!!! Please double check your wiring, diagrams can be found here

I stared like 10 minutes at his photo and didn't see the error, thanks to @jens55's remark, it all made sense 🙂

Completely glossed over that one. Should have looked at the diagram.

But my test asked to disconnect the bed wiring and test the terminals without an SD card present. Was that not actually carried out?

I guess then with vin being fed into the bed terminals it was also present on the actual win terminals. Of course.

@gnydick: No problem, I hope you didn't damage your board 😉

@Phaedrux: yes indeed, it all makes sense now 😛

Sometimes I wonder why you even bother asking for help here, you clearly think you know better than all of us.

@elmoret I'll be the first to admit when I'm wrong. I didn't insist or argue that it was correct or protest against sending photos. So what's your point?

@bearer wow. People make mistakes. At what point did I argue, disagree, or try to defend my mistakes in this post?

I know exactly what you're referring to. Solution is easy enough, just don't interact with me. In fact, I can make that happen for you.
