It does sound like a phase is disconnected.

@kenbigt, to run auto calibration when the calibration is a long way out (as yours is), temporarily increase the dive height as explained on the wiki page covering auto calibration.

What is the print surface? Reflective or glossy surfaces are not optimal. Dark, matte surfaces are best.

Try a higher dive height. M558 H10 to start to see if it improves.

Is your Z trigger height really 16mm?

G31 P450 X0 Y-32 Z16.00

How can a 16mm trigger height and a 3mm dive height work?

@kenbigt its not too powerful for the Duet - its just capable of getting very hot. Depending on how you use your printer and what you view of failing safe(r) is depends on if this is a probelm. many people use 40W heaters in aluminium blocks - if the heater is on full power and not controlled then it can get very hot.
